Philosophy and Rationale

In any hospital or healthcare center, regardless of type and size, medical records of patients are considered essential for the practice of scientific medicine. In fact, medical records form the very basis of deductive diagnosis. Hence, the discipline of medical records has to be very accurate, specific and specialized. The information contained in Medical Records serves as a tool for communicating with all the health personnel who deal with the care of the patient.

The number and types of health records are increasing, and many healthcare professionals are pursuing a central electronic location for them. As the healthcare industry continues to grow, more health records personnel will be required by hospitals, government agencies, pharmaceutical and insurance companies.

These personnel must be able to:

  • Use various classification systems to code and categorize patient information for reimbursement
  • purposes, for databases and registries, and to maintain and update patients' medical and treatmenthistories.
  • Organize and manage health information data by ensuring its quality, accuracy, accessibility, and security in both paper and electronic systems.
  • Analyze medical data, compile reports on this data and assign codes to health reports.

Individuals who will qualify for this Professional Certificate Health Records Management (PCert. HRM) will be able to enter health data into paper or electronic records, analyze data to control costs and improve care, or handle coding to aid reimbursement. They also ensure compliance with national laws, and improve the quality of the data entered.


The Professional Certificate in Health Records Management program typically takes 200 lecturer-student contact hours to complete. Students are advised to spend nearly the same number of hours on the take home assignments and further reading. The program is designed to provide students with an understanding of how to manage records and information in a healthcare environment. Students learn how to analyze, retrieve, store, classify and collect health information.

This program prepares students to update medical files and health records, analyze and maintain patient records and assign correct diagnostic codes using an established medical coding system. In addition to learning old-fashioned filing, record retrieval and transcribing techniques, Practicing Medical Records Officers will learn for the first time or refresh their knowledgein Anatomy and Physiology, Medical Terminology, Health Records management, the Law Relating to the Health Professions in Ghana Basic Healthcare Statistics, Standard Operating Procedure, DHIMS and Health GIS. Candidates will also develop the Communication Skills & Customer Care skills required in their career.


The duration of the program is Four (4) – Eight (8) weeks depending on the location and preference of participants. The basic requirement is that it takes a minimum of 200 hours to complete the program.

Objectives and Competencies

At the end of the course the trainee should be able to:

  • Operate and use computer and their peripheral devices
  • Use computer software to solve management problems
  • Effectively and efficiently process patient admission and discharge documents.
  • Participate in the regular review of records to ensure completeness, accuracy and compliance with the regulations.
  • Properly document important data such as demographic characteristics, history and extent of disease, diagnostic procedures and treatment modes using computerized means.
  • Prepare statistical reports, narrative reports and graphic presentation of health Information for use by hospital staff, researchers, etc.
  • Effectively audit Health records management practice and carry out an information survey of existing Health Records Management Systems.
  • Application of Geographical Information Systems in health facility location, spatial distribution diseases and patient records management.

Admission Requirements

  1. Must be a facility based Medical Records Officer (or sponsored by a hospital)
  2. Must submit a recommendation letter from the head of department in a health facility

Graduation Requirements

A student shall be deemed to have:

  1. 1. accumulated a minimum of 180 hours of personal contact with instructors and passed all tests.
    (Participants must attend 90% of personal contact with instructors and passed all tests).
  2. submitted a Long Essay or a Team Project Work
  3. submitted resume and personal 10 year game plan
